Educational Technology Certificate Programs

Documents and Forms

Below are the steps necessary to receive a transcriptable certificate for your graduate certificate:

  1. After being admitted to the MAET or graduate certificate program, complete the Intent to Enroll form. This form also serves as an academic plan between you and your graduate certificate advisor.
  2. Complete the three required courses.
  3. Once grades have been posted for all three courses, submit the Completion Review form. This will notify the program that you have completed the required courses and that you are ready to have the graduate certificate added to your transcript. You will also receive a printed Certificate from the University (this usually takes a few weeks to process).

Please note, you must complete the three required courses with a minimum cumulative GPA of 3.0 in order to be eligible for any of our graduate certificates. If you have any questions about this process, please contact the Graduate Certificate in Educational Technology advisors at
